You mentioned that a CCM phone can dial the Asterisk extension.  Do the
Unity
ports use the same calling search space?

If you use the port monitor tool, do you see Unity attempt to dial your
custom
extension?  Are any of the ports set to allow MWI out-call?

Another possibility is to use one of the message notification rules.

I haven't do this, so I may be wrong, but I think the flexibiity you
need
is in Unity.  It just may take a bit of poking to find the right
combination.
