Did you compile the samba sources by yourself with "--with-pam" or
"--with-pam_smbpass" ? If so, better recompile the latest version
2.2.6
without pam support, since you most likely won't need pam.
Optionally change the file "samba" in "/etc/pam.d" to
something like:

auth required /lib/security/pam_permit.so
account required /lib/security/pam_permit.so
password required /lib/security/pam_permit.so
session required /lib/security/pam_permit.so
Simplify your smb.conf and make sure "guest account" is set in both
"passwd" and "smbpasswd":
