Any recommendations on hosting providers (US) that offer a fully
managed dedicated server with rails support?

+1 on Rimuhosting. They''re a new zealand company, but their servers  
are located in the US.

I also know people that are very happy with layeredtech.com and  
bytemark.co.uk.

+1 on Bytemark.  They will give you a dedicated linux box with root 
access and you can install rails and whatever else you need yourself.

layeredtech is not managed. I''m not sure if rimu is managed or not.
