If you don't want to use R-patched, you need to download RHOME/src/modules/lapack/init_win.c, e.g. from https://svn.r-project.org/R/trunk/src/modules/lapack/init_win.c There was a corrected tarball posted, but I forget the name, and can't seem to spot it.
