A couple of things that first come to my mind:
1) strace the affected PIDs
2) Try to find patterns (if the time before 100% is always the
same, if something triggers this, if a number of users triggers
this).
The other thing is that *maybe* you are really under heavy
load, so you need to upgrade the hardware or downgrade the number
of users. ;)
