Rails - Sep 2005 - Retrying invalid statement with reopened connection

Hi
In one of our applications I got an error message
logged ''Retrying invalid statement with reopened
connection'' and it appears that the same input from a
form was inserted twice into the DB.

This behaviour made me verify the logs from the last
two weeks and I saw that it came up 4-5 times.

This seriously endangers the integrity of the DB.

What causes this behaviour/error?
