when appending an entry, it's sometimes useful to know in which place that
entry was inserted. Instead of returning 1 when the operation is executed
successfully, in this patch I just return that position.

it's a trivial patch but it helped us to achieve, in just one operation, to
append an item and to retrieve the position the item was inserted.
