Xen users - Dec 2010 - Any ideas why this lvcreate kernel BUG happens?

Hello,
From time to time I get this error and the lvcreate process becomes with D state
and can not be killed?
Any ideas why this happens? I use Debian Lenny with xen-4.0.1
