Broadvoice + incoming call works only for ~2 minutes

Broadvoice latest changes and still not working
I have added the three lines to the sip.conf file based on the latest changes from broadvoice. I can receive incoming calls but cannot place any outgoing calls. I've tried everything with the * box after this weekend. I have read every document on the problems people are having with them after this weekend as well, but none of them address my problem. I checked my settings in my sips which I have below as well, I have changed the host file a few times, but this was new to me and I never had modified it before. I have and the same results

Broadvoice problem
Since Thursday evening my asterisk box has been failing to register with broadvoice. I haven't changed any of my config files in the last week. Can anyone suggest anything? Will not authenticate incoming VOIP provider calls
I running Asterisk 1.1 on Mandriva 2006. Everything works fine, can connect with softphone, send outgoing calls to VOIP provider. Broadvoice latest changes and still not working- An Additional Server
I have been going crazy with this also since Sat. Our server was working perfectly with BV but will now not place calls to BV. Incoming from BV works fine. I felt sad rebooting it today, it had been running for almost 200 days! Here is my error message from the console... weird outbound problem through broadvoice (new)
Hello, Have a weird problem when using asterisk (1.0.6). There are certain numbers I cannot dial when using asterisk with my broadvoice account. No problems with inbound.
