Booting a Xen PVH guest requires a special boot entry as it is
mandatory to setup some Xen-specific interfaces rather early. When grub
or OVMF are used as boot loaders, however, those will fill the boot
parameters in zeropage and there is no longer a need to do something
PVH specific in the early boot path.

v1 was here:

However it was broken in a few ways. First of all the documentation
was broken because "/**" enhanced comments were not permitted on
macros. This is fixed in the new 1/4 patch.
Secondly we didn't use single_element() everywhere possible, which
is fixed in the new 4/4 patch.

[RFC libnbd PATCH 0/4] Add CMD_FLAG_DF support
RFC because this is an API break, but we haven't declared stable API
yet. If we like it, I'm working on using libnbd to implement the
nbdkit-nbd plugin; knowing whether it is API version 0.1 or 0.2 will
be useful. I also dabbled with allowing optional parameters in python,
although my OCaml is weak enough that there may be cleaner ways to
approach that.
