Karl,
does /home/dovecot/index/domain.com and/or
/home/dovecot/index/domain.com/user exist?
if these directories don't exist already it might be complaining about
trying to write a file in a non-existant directory heirarchy.
That's the only thing I can think of at the moment that might cause this
problem.
