asterisk users - Nov 2003 - [Asterisk-Dev] Help with Conference

Here is the scenario.

i have a meeme conference extension at 8000.

now i want to dial out and call tom (9000).
Once this call is established, i want to connect him to the conference 
extension, so that
he can participate in the conference.

How can i do this in asterisk? what is the sequence of commands to follow.
