Looking at the contrib/redhat/openssh.spec file
.....
%attr(0644,root,root) %config(noreplace) %{_sysconfdir}/ssh/ssh_config
%attr(0600,root,root) %config(noreplace) %{_sysconfdir}/ssh/sshd_config
%attr(0600,root,root) %config(noreplace) /etc/pam.d/sshd
....
we see those files are noreplace.
So some of the possibilities are a modified openssh.spec, or maybe using
--force or --replacefiles caused grief.
