Ok, it's taken me a while, but here's what seems to be the problem. In OSX I see a lock on the file. If that lock is ON, it disappears. But what is that lock and where did it come from.... I have some nice forms done using stylesheets. I currently have them displaying on screen and the user could simply just print. This works when there is only one form (facility) to be viewed/printed. You have the setting on to HIDE files that can't be written to. Try turning that off. Next, investigate why it's causing the files to get 540 permissions instead of 740 (or other).
