Postfix "userlookup" via Dovecot Auth
I'm hoping to take advantage of authentication caching via
dovecot-auth. I see that posfix can communicate with Dovecot-Auth, via
SASL, but from what I can see, postfix only does this for
authentication checks.
Not 100% sure how postfix handles virtual_transport (I assume it needs
to do a lookup via virtual_mailbox_maps), but I can't see anyway to
get virtual_mailbox_maps to use

Problem with DIGEST-MD5 authentication and plaintext passwords
I wasn't able to get DIGEST-MD5 authentication working with passwords
stored as plaintext until I tweaked password_generate() to make it not
use the domain portion of user at domain.com as the realm. Both evolution
and kmail send the email address as the username with no realm; when
dovecot tries to convert the username to user + realm, the hash is
different and so DIGEST-MD5
