PXE install in a boot on san with LVM environment, fails after reboot

I can install it using the ISO, with basic configuration. The
/root/anaconda-ks.cfg looks like the following (I striped some part, I
cannot copy past because I have only console access through a secured
VPN :-/ ):
firstboot --enable
ignoredisk --only-use=mpatha,sdc,sdd,sdb,sda
bootloader --location=mbr --boot-drive=mpatha
autopart --type=lvm
clearpart --all --initlabel --drives=mpatha
%packages
@base
@core
%end
My ks.cfg template is identical (I also cannot attach it
unfortunately?).
I tried several things like:
- put sda as boot-drive for bootloader (...

Expanding an LVM Storage Pool
...ng VMs or is the only affect that the pool is unavailable for management by libvirt?
These are the commands that I ran to create my pool and expand it after a kickstart of the server:
pvcreate -ff -y /dev/mapper/mpath?
virsh pool-define-as --name GuestVols --type logical --source-dev /dev/mapper/mpatha --target /dev/GuestVols
virsh pool-build GuestVols
virsh pool-start GuestVols
virsh pool-autostart GuestVols
virsh pool-destroy --pool GuestVols
vgextend GuestVols /dev/mapper/mpath{b..d}
vgdisplay GuestVols
virsh pool-start --pool GuestVols
virsh pool-info --pool GuestVols

CENTOS 7.2.1511 Multipath and boot from SAN issue
...N side, but I have an issue with multipath or udev or systemd, I'm not sure.
I when I boot my machine with the multipathd service enabled at boot, boot fails beacause some partitions on the multipathed device are missing from /dev.
>From the boot rescue shell, if I do a fdisk -l /dev/mapper/mpatha, I see 8 partitions, if I do a ls -l /dev/disk/by-uuid/ I see only 6 devices, so two devices are missing.
I have to run kpartx -a /dev/mapper/mpatha manually in order to reread the partition table and recreate the 2 missing device. Only after that I'm able to boot correctly.
Any idea of te roo...

The qcow is hosted on an ext3 filesystem.
$ findmnt /guestimages
TARGET SOURCE FSTYPE OPTIONS
/guestimages /dev/mapper/mpatha-part1 ext3 rw,relatime
Not sure why we are using ext3. Regardless, it seems user_xattr option is
not enabled. I imagine this must be enabled.
