Preliminary vcut patch for chained streams

are some cut positions near logical stream boundaries that can't be
encoded, but an error is shown in these cases (I haven't seen any
failures outside such ranges). I've uploaded 2 files that I used for
testing:

Updated vcut patch

I believe this version is ready for merging. It produces the correct
output for all cut positions in the vcut-test-tones file I mentioned in
my last message [1], except for positions in the last packet of a stream
(an error is shown for these).

Updated comment padding patches
I've attached updated patches (against vorbis-tools 1.3.0b3.5) to
support comment padding. No changes to libvorbis are necessary. The
padding is added in the same way as oggenc2 (null bytes within the
packet, at the end) and shouldn't cause problems with any reasonable
decoder or comment editor implementations.
