Ghost in the Channel-Banks

I've been struggling with an ongoing problem the last month.
Here is the layout of the wiring:
T1 from ISP > DiTech Echo Cancel device > Voice Channel-Bank
(same) T1 from ISP > (same) DiTech Echo Cancel device > asterisk1 server
zap card > fax channel bank
(same) T1 from ISP > (same) DiTech Echo Cancel device > asterisk1 server
zap card > asterisk2 server
Now, let me explain the symptoms....

D-channel errors and Channelbanks
...a T1 provided by Knology. We recently had an outage of Internet
and phones. The following problems (except for the log messages) seemed
to have cropped up after the outage. I was prompted by a friend of mine
to power cycle all the equipment detailed below (not sure if that's
relevant.)
T1 > Ditech Echo Canceler > Asterisk server asterisk1 > Zhone Z-plex 10
Fax channel bank > fax devices
T1 > Ditech Echo Canceler > Zhone Z-plex 10 Voice channel bank > voice
devices.
Now, let me explain the symptoms of the problem.

T1 Hardware Echo Can
Hello,
After reading the lists and taking reccomendations from TC, I have finally
given up on the echo can built into asterisk. I am sick of hearing
complaints from users, so the money spent on a hardware echo can will be
worth its weight in gold.
I am curious however, about some setup and component requirements. It
seems as if every telecom place I call, either never calls back or doesn't

TE411P Really Bad Echo
I just implemented a system using a TE411P hardware echo cancellation
card. Per Digium, I setup zaptel.conf, and zapata.conf the same way as
I always have. To my surprise calls out to the PSTN had a terrible
echo. 1 - 2 second delay, and quite clear. The echo was so bad that I
had to remove the hardware echo cancellation module from the card. We
are only using the 1st span of this card right

YET Another echo issue PRI CARD Any help accepted :-)
Good Day all,
I have a Fractional PRI connected to my Asterisk Box via a T100P
card.
When I initiate a call out to phone number 123-8888 the call
sounds great no echo what so ever.
If the person at 123-8888 hangs up and calls me right back (same
handset on both sides) same trunk line
