Rails App on Mongrel+Apache Stops Listening

[slightly OT] Apache 2.2.2 proxy config
...balanacer / mongrel_cluster / Mongrel on Mac OS X Server 10.4.5.
Typically the first upload would be fine but the next would make the mongrel
instance remain running (ps state = "R") essentially locking up that
instance. Apache would then eventually do:

something wrong with mongrel?
Hi all,
I''ve followed http://reductivelabs.com/trac/puppet/wiki/UsingMongrel
for configuring my puppet with mongrel.
Al seems to work fine, except that, after a reinstall of 40 nodes atone
time, I got many kind of errors like:
