necessary in most cases.
- Move the paravirt PV_CALLEE_SAVE_REGS_THUNK code to an asm header.
- Add a patch to collect PV qspinlock statistics which also
supersedes the PV lock hash debug patch.
- Add PV qspinlock performance numbers.
