Rails core - Oct 2013 - New CodeNotes gem, extracted from Rails rake notes

CodeNotes 0.0.1 -- Pull arbitrary notes from any set of files. An 
extraction of the Rails notes task

The gem is pretty cool. It runs from the command-line (or via rake) and has 
a few fixes from the Rails code.

I''m happy to work on turning this into a PR to remove the code from
Rails
and move code_notes into the Rails namespace, if there''s interest.

someone on the core team takes ownership of shepherding the code/me through
