           How can I extract survival probabilities from a fitted extended

Perhaps you should define the term "extended" in the context of Cox
models?

Ideally you would offer one of a) reference to a specific help page in a
specific package demonstating the constructions of such a model or b) use R code
to construct an applicable example.
