asterisk users - Jan 2009 - simple dial plan - brain dead operator

Suggestions for coming up to speed on simple dial plan.  I have not done them in
at least a year and my backup files seem not to have survived various PC h/d
migrations.

I downloaded visual dialplan standard, but it refuses to connect remotely to my
* box.  Even tho I can putty into it.

The sample extensions.dconf has an example IVR as well as a sample
extension or two.
