Hi Robert,
extensions.conf grammar is very easy:
To change 602085551212 to really dialing 6011442085551212:
exten => _602.,n,Dial,IAX2/user:passwd@pbx.address.com/6011442${EXTEN:3}
Everything that starts with 602 - take out the first 3 digits, and add
instead 6011442 before the number.
