asterisk users - Feb 2005 - how to add extension to mysql database

i want to know how we can add extension to mysql
database. i am using asterisk_addons and i have
checked that mysql database is connected with asterisk
here is trace

but when i try to make a call to 3000 it is answering
with 403 forbiden. can any one tell me what is problem
is there any problem in connection with mysql or any
problem in  mysql tables
