Rails core - Mar 2008 - Patch review request for FormOptionHelper respecting default options :index => nil

Hi,

This patch is related to [8786] where you can set default options on  
form_for and fields_for. This patch lets FormOptionHelpers like select  
and country_select to use the default options set on form_for and  
fields_for.
