There is a spelling mistake in the switch statement
"extentions"
But try to use different names in extconfig.conf
for the switch and table i.e.:
exten_sions => mysql,asterisk,extensions_table
or something, and then
switch => Realtime/default@exten_sions
to avoid problems.
