Write the question in a label. Put a groupbox in your form. Put 4-5 radiobuttons inside the groupbox in which you'll type your multiple answers. The user will check one of the radiobuttons. Write if statements checking which radiobutton is selected and assign scores or "true, false" values to them. This is only a clue. You can do the rest.
