blk-mq crash under KVM in multiqueue block code (with virtio-blk and ext4)

we have seen the following bug with 3.16 as a KVM guest. It suspect the blk-mq rework that happened between 3.15 and 3.16, but it can be something completely different.

we have seen the following bug with 3.16 as a KVM guest. It suspect the blk-mq rework that happened between 3.15 and 3.16, but it can be something completely different.

I''ve got similar problem - after HW failure - two of three disks became unavailable and now I''m unable to btrfsck.
Btrfs lays on /dev/sdb1,/dev/sdc1,/dev/sdd1 - sdc+sdd became unavailable

The amd64 Dom0 crashes in early boot-stage. For debugging purpose I logged Kernel-Dump
with minicom over Serial Console:

Dell Poweredge R710
2x Intel XEON X5650
96GB RAM
Perc H200 SAS Controller
3x SAS-Drive
I see a possible conjunction with Bug #600241 but acpi=off doesn't solve this
problem.
