Clean up of nv40_context->state.hw and nv40_screen->state

Currently on NV30/NV40 an assert will be triggered once 32 queries are outstanding. This violates the OpenGL/Gallium interface, which requires support for an unlimited number of fences. This patch fixes the problem by putting queries in a linked list and waiting on the oldest one if allocation fails. nVidia seems to use a similar strategy, but with 1024 instead of 32 fences. (patch) Gallium NV50: honor bypass_vs_clip_and_viewport
When trying out the Gallium3D NV50 driver (curiosity) with a small OpenGL program that renders 2 rotating triangles partially occluding each other I noticed that depth buffer clearing by rendering a quad (st_cb_clear.c/clear_with_quad) didn't work properly. [PATCH] gallium/nv50: fix multi-texturing
This one maps textures to sampler units (or textures to texture units or whatever it's called), which wasn't done before. It should make the mesa demo "multiarb" work, at least with the shader patches I sent earlier. Of course, with this functionality one probably wouldn't have to setup the textures in NV50_TIC anew every time, but that can be optimized later.

This moves construction of the mapping between VP outputs and FP inputs into validation. We need to invert the viewport translate/scale parameters when the state tracker thinks we have Y_0_TOP. If these cases, we have do to bypass mode by setting an identity viewport transform for x, z and inversion for y, or p.e. clear_with_quad won't work correctly. [PATCH] nv04-nv40: Rewrite and unify miptree and transfer code (v2)
Changes: - Fixed some nv##_miptree that were not converted to nv04_miptree. - Disable swizzling on non-RGBA 2D textures, since the current 2D code is mostly broken in those cases.
