sshd should definitely not be using 'NOUSER'. The correct thing is to use
the username, regardless of whether (pw) exists. I can't understand why
you would substitute the value 'NOUSER'.

OpenSSH v6.7 & NumberOfPasswordPrompts Option ...
So it appears that I am getting a keyboard-interactive prompt and then a password prompt.
Here is the output of the requested command:

[Patch] PAM Service name option
It's sometimes desired to be able to alter login policy depending upon
how the person was connecting for the ssh server. For example you might
want different rules on the internal and external interface of a
gateway. In another setup you might want an sshd with a different login
policy running on a different port - and setup different firewalling

OpenSSH v6.7 & NumberOfPasswordPrompts Option ...
Yes, I have tried that option with no difference in behavior. It seems it ignores that option when provided. Just for reference, I am building it on RedHat 5. I have never had this issue on any previous version of OpenSSH. I use the default configuration with only the changes specified in the RHEL 5 STIG applied.

I discovered an issue in the latest version of SSH, where the number of password prompts are doubled. If I specify 1, I get 2, and so on.

OpenSSH PATCH: OpenCommand and CloseCommand
Hi,
the attached patch adds support for the keywords "OpenCommand" and
"CloseCommand" to ssh_config. They are commands which are executed before
the connection is established (or ProxyCommand started) and after the
connection has been closed (or ProxyCommand ended).
this is usefull for stuff like portknocking or (that's what I wrote the
patch for) talking with trapdoor2

The firewall at work doesn't allow me to make a direct SSH connection
to the Internet, so I use the ProxyCommand to tunnel SSH through a
HTTP proxy. This works fine, except for the fact that the HTTP proxy
server closes the connection after 60 seconds of inactivity. Attached

dovecot-lda, sieve, maildir
I'm unable to get dovecot-lda with sieve filtering to deliver into
maildir folders. The examples on the wiki explicitly say "mbox", so
I'm wondering, does the dovecot-lda sieve implementation not support
filtering into maildir folders?

[PATCH] Added Null packet keepalive option
I have attached a patch which adds null packet keepalive
functionality to the client. This patch is made against the
current CVS tree as of 3/14/01.
Please consider this patch for inclusion in the OpenSSH main tree.
This patch is based upon and includes code from the Chris Lightfoot
