Trouble with Realtime

Under extensions.conf, in the [from-sip] context:
switch => Realtime/from-sip@extensions.
Running isql MySQL-asterisk voip <pass> connects to the DB, and
queries return the proper data.
I have the following tables in the mysql databases:
